Setting up a Private Health Services Plan (PHSP) is a simple 4 step process:

1. Download the Enrolment Form. Please choose the form based on the number of employees you are enrolling and also select the form based on company structure: For example, sole proprietor, or Incorporation .

2. Print off and fill out the form (s). Be sure to sign it.

3.The one time registration fee* for your PHSP is $335 plus $40 per employee (fee covers family members) plus GST.

Please Enclose a cheque drawn on your business bank account in Canada for the $335 + GST Enrollment Fee made payable to Olympia Trust. Please add #40.00 per employee enrolled in the plan.

* Registration fees are also 100% tax deductible.

4. Planholder mails the forms and cheque to:
